Add a Custom Field to the Update Subscriber Form

Sometimes you need to add a custom field for subscribers (in the Leaky Paywall dashboard), that you can manually add values for some of your existing subscribers.  

Here is an example that adds a "company" custom field to the Update Subscriber from when editing a Leaky Paywall subscriber from Leaky Paywall > Subscribers.
