Add Terms and Conditions Checkbox to Registration Form

Depending on where your business is located, you may be required to add a Terms and Conditions checkbox to your registration form, .  The following code snippet lets you add the checkbox to the registration form.  

It will show up before the "Next" button for paid levels.  You can also adjust the text based on if the level is recurring or non-recurring.
